Nationwide Transdiagnostic Age and Sex Patterns in Mental Disorder Disease Development

This Danish nationwide registry study followed 273,400 adults (55% female, mean age 41) for 2.1 million person-years after their first psychiatric contact, tracking whether their diagnosis shifted category over time across seven disorder groups. Nearly a quarter of patients (24.3%) received at least one diagnostic shift, and 11.8% had more than one. Younger age at first contact predicted more diagnostic shifts and more subsequent psychiatric contacts, regardless of the initial diagnosis. Among patients who did shift, men moved disproportionately toward substance use and schizophrenia-spectrum diagnoses, while women moved toward mood, anxiety, and personality-disorder diagnoses. Patients whose first diagnosis was psychotic had the highest overall contact and diagnostic-shift rates of any group.

Cognitive Rehabilitation and Functional Outcomes in Long COVID–Related Cognitive Impairment: A Randomized Clinical Trial

This UK multicenter trial randomized 78 adults with persistent long-COVID brain fog to 10 weekly telehealth sessions of goal-directed cognitive rehabilitation or usual care. At 3 months, the rehab group scored significantly higher on a standardized goal-attainment interview than usual care (adjusted difference 2.88 points, Cohen d 1.57), and the gap remained significant at 6 months though it narrowed (difference 1.72). Anxiety, depression, and fatigue scores did not differ between arms at either time point β€” the benefit tracked with functioning toward self-chosen goals, not with mood or symptom burden. The trial was single-blind with a self-reported primary outcome, so some of the observed effect likely reflects expectancy rather than a genuine change in underlying cognition.

For patients whose cognitive complaints persist despite adequately treated mood and anxiety, structured goal-directed rehab is a reasonable non-pharmacologic option to offer β€” but the unblinded, self-reported design means the effect size shouldn't be taken at face value.

Valproate's total level is lying to you when albumin drops

Standard valproate levels measure total drug, but only the unbound fraction β€” normally about 10 percent β€” crosses into the CNS and drives both effect and toxicity. Valproate binds almost exclusively to albumin, and that binding is saturable: as concentration rises, more drug gets displaced free even while the total stays inside the therapeutic range. Anything that lowers albumin β€” cirrhosis, nephrotic syndrome, malnutrition, late pregnancy β€” or competes for the same binding site β€” aspirin, warfarin, other highly protein-bound acidic drugs β€” raises the free fraction without moving the total level at all. The failure mode: a hypoalbuminemic or pregnant patient with a normal total of 70–80 mcg/mL who is nonetheless ataxic, encephalopathic, or thrombocytopenic, because their free fraction is running two to three times higher than a patient with normal albumin at that same total. Order a free valproate level whenever the clinical picture doesn't match the total, and treat a reassuring total with real skepticism whenever albumin is low.

Withdrawal ruptures and confrontation ruptures need opposite repairs

Alliance ruptures come in two structurally different flavors, and treating them the same way is why repair attempts fail. A withdrawal rupture is the patient pulling away from you, the work, or their own affect β€” intellectualizing, going vague, conceding too quickly, repeating 'I don't know.' A confrontation rupture is the patient pushing at you directly β€” criticizing the treatment, expressing anger, testing whether you can tolerate their hostility without retaliating or over-apologizing. The technical error is symmetric: meeting withdrawal with more exploration accelerates the retreat, because the patient reads probing as more of what made them withdraw in the first place. Meeting confrontation with premature repair forecloses the very affect that needed airing. Withdrawal calls for naming the retreat and slowing down; confrontation calls for staying inside the disagreement, not soothing it away.

The instinct is to ask a deeper question about the mother; better technique names the shift itself: something changed just now, right when we got close to your mother β€” can we look at that instead of pushing forward? The patient goes quiet, then says she's afraid that talking about it means blaming her mother, which is what she's spent years refusing to do. The rupture was data, not a stall β€” naming the withdrawal opened the material the exploration had been blocked from reaching.
