Life's Essential 8 Cardiovascular Health and Suicide Mortality: Findings From a Nationally Representative Cohort in South Korea

Researchers followed 54,382 Korean adults from a nationally representative health-screening cohort, matched to the national death registry from 2007 to 2022 (median follow-up 9.67 years). They calculated Life's Essential 8 (LE8) cardiovascular health scores โ four health behaviors plus four biometric factors, scored 0 to 100 โ and used Cox regression to relate baseline LE8 category to death by suicide.
Suicide mortality was 0.28 per 1,000 person-years overall, but adults with a poor LE8 score (0 to 49) had 2.49 times the hazard of suicide death compared with those scoring in the ideal range (95% CI, 1.06 to 5.82); the intermediate group's risk was not statistically distinguishable from ideal. Notably, the behavioral components โ diet, activity, sleep, nicotine exposure โ tracked with suicide risk more strongly than the biometric ones (blood pressure, lipids, glucose, BMI).

Obsessive-Compulsive Personality Disorder in Radically Open Dialectical Behavior Therapy for Treatment-Refractory Depression

This secondary analysis of the multi-site RefraMED trial randomized 250 adults with treatment-refractory depression to Radically Open DBT plus treatment-as-usual or TAU alone, then asked whether comorbid obsessive-compulsive personality disorder (OCPD, present in 117 of 250 participants) changed how much patients benefited. At 12-month follow-up, RO DBT recipients with OCPD showed significant gains over TAU in emotional approach coping and psychological flexibility (p-FDR = 0.008), and their outcomes were statistically indistinguishable from RO DBT recipients without OCPD across every measure at both 7 and 12 months.
OCPD diagnostic status did not moderate treatment response on any outcome โ the rigid, perfectionistic, overcontrolled presentation that clinicians often assume needs a different approach did not blunt response to a therapy built around openness and flexibility.

Stopping the patch doesn't stop the receptor
Clonidine and guanfacine don't just wear off when stopped โ abrupt discontinuation triggers a rebound sympathetic surge: down-regulated presynaptic alpha-2 autoreceptors stop restraining norepinephrine release, and plasma catecholamines can spike above pretreatment levels within 24 to 72 hours, producing tachycardia, hypertensive urgency, agitation, and a withdrawal picture that can mimic pheochromocytoma. The trap is a patient also on a beta-blocker: with alpha-1 vasoconstriction now unopposed by beta-2 vasodilation, the same catecholamine surge produces a sharper, more dangerous pressor response than in a patient on no other cardiac medication. This bites hardest on transitions clinicians treat as routine โ converting a patch to oral dosing, a discharge reconciliation that drops a home dose, or a patient who simply runs out. Taper both drugs over days, never stop either abruptly, and if a beta-blocker must continue, taper the alpha-2 agonist first or alongside it โ never leave it stopped while the beta-blocker runs alone.

Apparent competence: the DBT skill that fools everyone in the room
DBT's apparent competence dialectical dilemma names a specific clinical trap: many patients with intense emotion dysregulation can perform composure convincingly in one context โ a phone call, a family gathering, a discharge interview โ while remaining acutely at risk minutes later. This isn't deception; it's mood-dependent behavior, driven by state-dependent learning and a lifetime of reinforcement for looking capable. The error belongs to the observer, not the patient: clinicians, family, and even the patient's later self extrapolate stability from one competent performance into contexts where it doesn't generalize. A DBT clinician treats apparent competence as a standing hypothesis to actively rule out, not a reassuring default โ asking context-specific behavioral questions rather than inferring safety from tone or how "together" someone sounds.

A resident calls a patient in crisis; she describes her plan calmly, uses the coping skills she's been taught, and sounds, by the end of the call, entirely stable. The resident charts "no acute risk, redirected to skills" and signs off. That night she overdoses. The technical move DBT adds: instead of inferring risk from tone, ask a context-anchored question in the moment โ "on a 0 to 10, what's your urge to hurt yourself right now, regardless of how I sound to you" โ and cross-check against her diary card, not her voice. Competence on the phone was real, and it told the resident nothing about ten minutes later, alone. |
